Hello and thank you for your question and the picture. For starters, I'm so sorry to hear about what you went through, that's absolutely awful! Addressing a crooked or asymmetric nose (especially if you have a history of nasal trauma) is actually one of the more common reasons people seek out and undergo a rhinoplasty. An open septorhinoplasty can best address both your aesthetic concerns and any functional issues with your nose. I would be able to address your septum, improve your breathing, straighten your nose, and also adjust the size, shape, and definition of your nose!

This truly a surgery of millimeters- and whether you're looking for a minor adjustment or a major transformation, it requires exceptional surgical skill to achieve functional and natural-looking results! Because your nose is the DEFINING feature of your face- I would only recommend seeing a surgeon who specializes in rhinoplasty procedures, and who has significant training and experience performing these surgeries. I actually trained under one of the world leading rhinoplasty experts (who performs 1,000+ nosejobs annually).

I'm truly sorry to hear about the sequence of events that you have experienced.

One aspect to consider in the evaluation of your current situation versus Photoshop projection is the interval between surgery and present day. Since it is not yet one year from your operation it is quite likely that there is still swelling affecting the width of the nose which will continue to improve over the next year.

If you are contemplating further surgery to produce a more refined appearance of the nose, it is important to appreciate that it does not restore the original shape. This is accurately reflected in your Photoshop simulation and the simulation is reasonable in terms of the characteristics of the nasal skin suggested by the photographs as well as the overall balance with the facial features.

The problems related to pain after injury to the nose are more complicated and not necessarily solved by more surgery. The recommended approach is to have a consultation with a pain specialist before embarking on further surgical interventions. This is because the type of pain can be analysed and it might lead to the conclusion that more surgery could make it worse. It may also be the case that the program of pre-treatment would increase the likelihood of surgery progressing without worsening of the pain.

Although the pain can be improved by surgery, it is not through a mechanism that we understand and therefore not something that surgeons can project on and this is an important aspect since pain is a constant reminder of the events that led to it.

Psychology help in situations like this is a valuable contribution and together with pain management and possibly surgery produce a holistic treatment that is more likely to be successful.

I would approach this as a standard rhinoplasty. Meaning refine the tip, lower the bridge a bit and break the bones to narrow the bridge. Internal works such as a septoplasty is also likely

Based on these photos you ended up with a decent result. You should wait at least one year after the last surgery before undergoing new rhinoplasty. In-person consultation with a board-certified plastic surgeon, an expert in rhinoplasty, is recommended. Good luck.

The most important variable regarding your candidacy for an elective post trauma rhinoplasty is going to be based on how well the repairs were done after your injury.
If your surgeon at the time was able to give you an atomic realignment of bone, Cartlidge, and soft tissues, then this would give you the best chance of a quality outcome from another liked, and rhinoplasty post trauma.
There’s a lot of information required to make an assessment, including the timeframe of when you had the repair of your injury. Also require a careful in person examination.
Most plastic surgeons will want you to wait at least 12 months from your last procedure or intervention.
Secondary rhinoplasty, whether from previous trauma or previous elective, surgery is going to be complex and potentially difficult.
If the repairs were done well, then increase the chance of having a quality outcome, coming elective at secondary rhinoplasty
Secondary rhinoplasty surgery is complex, inherently difficult, and should only be attempted by plastic surgeons who have extensive experience with rhinoplasty surgery. Look for plastic surgeons, who have strong practices with rhinoplasty surgery or preferably focus exclusively on rhinoplasty surgery.
