In December I was thrown face first to concrete in a domestic assault. My nose was completely fractured in many spots, very deviated, and cut on the right side. I had a reconstruction done. But I hate my nose, and it is still in pain. I want the nose I used to have back. I would like my tip to be back to refined, my bridge fixed(skinnier and not crooked), and my nostrils not protruding. One aspect to consider in the evaluation of your current situation versus Photoshop projection is the interval between surgery and present day. Since it is not yet one year from your operation it is quite likely that there is still swelling affecting the width of the nose which will continue to improve over the next year. If you are contemplating further surgery to produce a more refined appearance of the nose, it is important to appreciate that it does not restore the original shape. This is accurately reflected in your Photoshop simulation and the simulation is reasonable in terms of the characteristics of the nasal skin suggested by the photographs as well as the overall balance with the facial features. The problems related to pain after injury to the nose are more complicated and not necessarily solved by more surgery. The recommended approach is to have a consultation with a pain specialist before embarking on further surgical interventions. This is because the type of pain can be analysed and it might lead to the conclusion that more surgery could make it worse. It may also be the case that the program of pre-treatment would increase the likelihood of surgery progressing without worsening of the pain. Although the pain can be improved by surgery, it is not through a mechanism that we understand and therefore not something that surgeons can project on and this is an important aspect since pain is a constant reminder of the events that led to it. Psychology help in situations like this is a valuable contribution and together with pain management and possibly surgery produce a holistic treatment that is more likely to be successful.
