Skin Changing - What's happening with one side of my nose? (Photos)

I've had Rosecea for 30 years and have noticed skin changing for about 10 years on one side of my alar cartlidge. Not sure if it's skin or structural. Any idea what might be happening here? On and off mild pain, stinging / throbbing. It's annoying.

Based on your photos it appears you've had rhinoplasty with alarplasty. While what you describe may be possible, it may not be the best solution to optimize the symmetry of your nose as other factors come into play regarding the position of your alar rim - such as the orientation of your lower...

Based on your photos the asymmetry you're noticing is due to asymmetry of cartilage within your nose - lower lateral cartilage and caudal septum. Without addressing that structural asymmetry - requiring a rhinoplasty - it's not possible to significantly improve the symmetry of your nostrils.

This is a fantastic question to discuss with your surgeon as I'm sure she/he has a plan in place and there is a reason why you chose her/him to perform your procedure. Without seeing your base view and examining you in person, it's impossible to say whether 5 mm is appropriate or excessive.

I think it would be a mistake trying to correct your alar/nostril asymmetry with alarplasty alone. The shape and height of your alar rims is asymmetric because the underlying tip cartilage (lower lateral cartilage) is asymmetric. You can see the asymmetry of your tip from the frontal view photo...
