Looking at the photos you have provided I think your a great candidate for a breast augmentation with a silicone gel implant along with a breast lift. This will allow your surgeon to increase your size, give you better shape, fullness and the lift will help with the perkiness. Yes, you can! Breast implants under your muscle would be the best option because it doesn't look like you have much tissue at the top of your breasts. Trying on implants to see size and fit would be the best. Seeing a board certified Plastic Surgeon would help discuss the best options of implants for you!
You are a poor candidate for breast augmentation but an excellent candidate for a lift. The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. As you noted, you have deflated-appearing breasts after your weight loss. You would be a candidate for implants; however, to achieve the best aesthetic outcome, you would also need a breast lift. The implants alone will give you more volume and fullness of the breasts but would not address the amount of extra skin you have. Dear Thriving70212,determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift.
