Deep wrinkles and smile lines can be improved with a combination of Botox and/or fillers for the lines themselves and to build anatomical support with Sculptra for collagen building. Ultherapy is a great skin tightening device, but works best in combination with other treatments like J Plasma, RF treatments, lasers, microneedling/PRP, salt facials, Cosmelan for skin tightening and improve skin tone. Long term use of fillers in the skin will not stretch out the skin to make it the affected area look worse. If you are getting fillers for this concerns seen on the photos, I would recommend that you consider injecting fillers in the cheeks (to lift the smile lines) as well as inject along the smile lines to soften the indentations and smooth out the valleys..
The use of fillers will not cause long term changes to the skin. Once a filler has resorbed, the skin and underlying structures return to their baseline appearance. Based on the photos, you might want to consider facial skin resurfacing with a CO2 lase, chemical peels, or mocroneedling.

Looking at the picture I would recommend stimulating your collagen, elastin and hyaluronic acid with more natural treatment like profound RF so you get the results you want. Fillers will do the trick but you will have to do regular treatments and will not change the quality of the skin.

The use of fillers will not cause the skin to stretch or the wrinkle to get worse, but it truly won't help the skin and only temporarily makes the wrinkle look better. An option you might want to consider is the use of PDO Threads for both volume and lifting to help with your smile lines. PDO Threads can be used for nasolabial folds, marionette lines, cheeks to provide both volume and lifting to those areas with minimal downtime. Hyaluronic acid is a natural component of healthy skin, production drops as we age and we can enhance hyaluronic acid levels in our skin with most commonly available fillers. Your injector will likely start with revolumizing your cheeks, which will lift the smile lines and then may choose to place a little filler in the region of the smile lines themselves. Hello there, I actually would not use filler to address this area. I would instead use a laser or micro needling treatment to address the skin itself. This should stimulate collagen in this area, softening the fine line without using filler. It also will address textural issues/scarring with the skin, stimulate collagen and elastin, address any hyperpigmentation, and soften pores. I hope this helps. Sarah RN BSN
