Its been 3 months since the fat transfer in cheekbones and lips and i have a touch up in a month. The lips are gone. Would a second fat transfer to the lips be more successful?

Fat transfer to the lip is somewhat unpredictable in terms of long-term survival. The lips and mouth tend to move considerably -- chewing, breathing, speaking! I rarely perform fat transfer to the lips as a stand-alone procedure, although I will place fat in the lips for patients who request it, as part of a full-face fat transfer procedure. For more durable results, I have been performing PermaLip implantation for the last few years. It is a permanent solution for lip enhancement. It can be performed as a same-day office procedure.

Since the lips move a lot the energy they create can melt away the fat. I suggest trying a hyaluronic acid into the lip it is more predictable and long term more stable yet still reversible

This is a good question. I no longer perform fat augmentation to the lips because the "take" of fat in this area is unreliable and can often leave you with asymmetries or underwhelming results. For the last two years, I much prefer to use a permanent lip augmentation product called PermaLip. This is a very soft silicone implant that comes in various sizes which can be placed into the lips under a local anesthetic. The results are very natural and my patients love it. How do you know if you are a candidate? If you already know you like fuller lips then its a no-brainer, however, if you are unsure, I always suggest having filler placed first to make sure you like it and then you can have the implants placed once the filler goes away. Ask about this procedure instead. Good luck to you.

The predictability of fat transfer is different from individual to individual. Fat has risks such as clumping, uneven resorption and asymmetry. We have a technique to break down the fat into a very smooth paste, making it less viscous and more even. This allows more accurate placement of the fat with less clumping. In theory, it should increase the amount of fat staying behind, however, that needs to be determined by studies. We usually recommend fillers initially if you haven't tried it already. Fat should stay permanently after the initial resorption. Best of luck.
