I only want to remove a hump on my nose, leaving the nose tip the same. I was told that once you get a hump removal, the whole nose will have to be changed because the excess skin will have to be cut off in order for it to not hang lose on the tip. Is this true or can the hump be removed without having to touch the tip? also, what would be the costs of the hump removal? and is it full anesthesia or can it also be local?

Answer: Hump removal Hi,The nasal hump CAN be removed without altering the nasal tip components. There is almost never a need to remove skin when performing a rhinoplasty. The only other important component of a hump removal is the need to perform osteotomies - which are controlled fractures of the nasal bones to narrow the nasal bride and prevent an "open roof" deformity. I recommend visiting with a rhinoplasty expert to discuss your goals and to review all the details of surgery including cost and recovery.Best,Mehdi Sina, MD

Answer: Partial rhinoplasty Dear Mari, Front and side view pictures would have been helpful. Apparently you have been told incorrectly about nose surgery: 1. Skin is almost never removed during rhinoplasty. Only if the tip projection is reduced significantly, a small amount of skin can be excised from the columella. Also, ,when the nostrils are too wide , a wedge of skin is excised from the alas' curve and base, to narrow the nose.2. The hump can be remove without changing the tip, However, usually, the nasal slopes have to in- fractured to avoid 'open roof deformity' . Also, if you have septal deviation and turbinates hypertrophy, the in-fracturing of the slopes can block the nasal breathing, so septoplasty and turbinectomies are recommended. You have to realize that not all results are the same, because experience, technical skills and aesthetic eye are critical for good outcome. So, do your due diligence carefully and choose your surgeon wisely. Always, consult with experience board certified plastic surgeons who operate in accredited surgery center for your safety. Most importantly, check the before and after pictures , to mak sure that they are numerous, consistent and attractive. Best of luck, Dr Widder
