Hello several years I had a facial fat graft and my left cheek became unnaturally large but right cheek is normal. I don't know if it's simply too much fat, calcification of fat/granuloma/ scar tissue but my upper left cheek is big bulgy and looks horrible when I smile. Picture is not accurate as it is very prominent. Can I be safely reduced with 5fu and very dilute steroid injection or what about microlipo or excising some fat? What is the best approach. I need an expert to help me.

Answer: Revising facial fat transfer I generally do not recommend people use fast transfer to argument The face. Compared to fillers fat grafting is unpredictable, and precise and unforgiving. Steroids and five fluorouracil will not reduce subcutaneous fat. Steroid injections as a very rare side effect causing fat atrophy. it is uncommon and when it happens it’s devastating. you can’t have a little fat atrophy by injecting a little bit of steroids. anybody who gets steroid induced fat atrophy well wish they never had steroid injections. To start with we need to make an accurate assessment. To do that we need to see proper before and after pictures. Even if this hasn’t been a few years we still need to know what you look like before you have the fat transfer. we also need to compare your left and right sides. If you don’t have proper before and after pictures and contact the provider who did your procedure and ask them to forward the pictures they took. While you’re at it you should also ask for a copy of the operative report.Depending where the area of concern is Liposuction may be the best approach him. Liposuction can however cause some temporary facial nerve damage. plastic surgeons have a good understanding of facial anatomy and where the branches of the facial nerve run. There may be other solutions to your problem but we can’t really make an assessment based on the single picture you include. I suggest you follow up with the provider who did your facial transfer. Here she is responsible for the outcome of the procedure. If you want a second opinion consultations then schedule those as in person consultations with other local board-certified plastic surgeons who seem to have a lot of experience with facial fat transfer. Facial contour comes from a number of variables. not all of them are related to fat distribution. Underlying bone structure, soft tissue coverage and tension on the skin from muscles or facial expression all contribute to the contour of the face. This is especially true areas around the mouth. Removing fat may or may not deliver the intended outcome. This becomes more likely if tension from muscles a facial expression are contributing. There is no easy way to remove fat grafting in the face and this is why I generally steer people away from the procedure or recommend patients take a conservative approach with having a second or third round of fat transfer and build up volume slowly if they opt for bed transfer as a treatment option.
