I had jaw surgery to correct an underbite 3 years ago but I developed severe nasolabial folds. I am early 30’s and this is affecting my mental health. I tried a facial fat transfer a year ago but it seems to have made them worse.

A mini facelift can lift the cheeks and correct the deep nasolabial folds.  It can also be designed to just address the cheek ptosis and and not other areas like the neck and therefore make for smaller incisions.

Thank you for your query. Based on your photographs and your age, injectable fillers may be your best option. I would not advise surgery at this time although a mid-face/cheek lift can help. A facelift for  your nasolabial folds would really be excessive and probably not do much anyway. I don’t think your fold are bad enough to even treat . However an option would be injectable fillers instead of surgery

Being in your early 30's, I do not believe many surgeons would be willing to perform a facelift. An alternative to consider would be a lip lift. However, most would be taken laterally and little centrally. The lateral lip lift can pull up the corners. This may help with the marionette deformity as well as the nasolabial folds.

A standard facelift does not typically improve nasolabial faults and can make them worse. Most plastic surgeons pull two layers of tissue when doing a facelift. The deep layer is referred to the SMAS or superficial musculoaponeurotic system.
This tissue layer is a continuation of the platysma muscle and creates a hammock. The vector on the pulling of this tissue, usually more upwards or superior posterior. The SMAS is attached to the nasal labial fold and pulling on this tissue air deepens the fold. For this reason, facelift results are very different than you simply pulling on your skin.
The best treatment for nasolabial folds is the use of fillers. Fed transfer is not particularly effective for precise volume in small areas, but works better for diffuse overall volume. I would not expect fat transfer to have given you a good outcome. Try working with one of the best filler providers in your community and see what fillers can do before, jumping into Big operations that have serious consequences.
