Hello, I am a 36-year-old female with severe tear trough depression. No matter how much makeup I wear, my face still looks exhausted and unhealthy due to the shadowy appearance and dark circles. I am seeking advice on the best procedure to permanently fix this issue. How difficult will it be to permanently decrease the hollowness under my eyes and diminish the appearance of dark circles? Will I need several procedures due to the severity of my case?
Answer: How to minimize tear trough depression? Fat grafting will decrease or eliminate the depressions on a long term basis but might need more than one treatment. The darker circles unfortunately are not treatable other than the use of makeup. However correcting the depressions will minimize the shadows for indirect lignting . Makeup will help , but you do not have excess fat so a lower lid blepharoplasty is unlikely to help.

There is no one best procedure. Each has benefits and drawbacks and depends on desired outcome and individual anatomy. In a patient with your individual anatomy, a temporary hyaluronic filler like Restylane would be a reasonable place to start, while a conservative artistically performed blepharoplasty and micro-fat grafts to the tear troughs will produce the best and longest lasting cosmetic result.. Following the advice of anyone who would presume to tell you what to do without seeing photos and without taking a full medical history, examining you, feeling and assessing your tissue tone, discussing your desired outcome and fully informing you about the pros and cons of each option would not be in your best interest. Answer: Lower blepharoplasty Lower transconjuctival blepharoplasty possibly combined with periorbital fat grafting would be a good procedure to correct your problem. In person or teleconference with a board certified plastic surgeon experienced in this procedure is recommended. Good luck.

Answer: Making an assessment Your pictures are insufficient for us to make a quality assessment. We need to see standard facial pictures without facial expression. When you smile, you’re lifting your cheeks and that distort the area under your eyes We typically make assessments for surgical intervention based on what people look like in a static state without facial expression.
