Is eyelid surgery and a cheek lift/mid face lift a good solution for my sagging skin? (Photos)

you could do an upper bleph alone but doing a browlift may be helpful to get a longer lasting result bc you'll have to do the brows at some point and you're kind of borderline already a candidate for that. as for the lower eyelid/cheek area - this is almost certainly because you have a deficient maxilla based on your photos, so while YES you can pursue a lift, you almost definitely need to get some voluem (fat grafting) too to hold things up. the lift won't be enough on its own and is likely to fail.

Hello, I would love to help with answering your questions. You would be a perfect candidate for both an upper and lower lid blepharoplasty. Along with the lower lid blepharoplasty, which involves the fat pad removal that sits beneath the eyes, I would then go in and re-inject micro fat into the area to help fill in the hollow areas and give you an evenly smoothed appearance. As far as the lower face, I do notice that you have some drooping that is making the appearance of the jowls and nasolabial folds more prominent. This would be resolved with a face lift. A facelift itself will only address the area of the face that stems from the jawline up to about the corners of your eyes. I do what is called a deep-plane facelift which is the most aggressive but also the most natural. This will reposition all of the fat that has migrated downwards back up to the desired location which will in return create more defined cheekbones.

I think you have a number of options based on what is most bothersome for you.
It appears that you have a combination of fullness of the lower eyelids from herniation of orbital fat combined with descent of the fat pads of the midface which accenuates the "hollow" appearance. For this, a lower blepharoplasty with fat repositioning and/or fat transfer to the upper cheek/tear trough would likely help significantly. This could be performed in isolation or with a facelift with midface lift based on your preferences.

Based on these photos you seem to have mild malar hyperplasia (small cheek bones) which is frequently the reason for an early onset of periorbital aging. A combination of lower and upper blepharoplasty and periorbital fat grafting (to augment the cheeks) would be my suggestion. Good luck.

At 37 years old, I recommend an upper and lower blepharoplasty. Your eyebrows appear to be in a good condition. Your forehead does appear somewhat high and may benefit from a coronal brow lift. At your age, I would recommend you start with the blepharoplasty with fat transposition.
