From your photograph, it seems like you would do very well with just an extended tummy tuck and liposuction of the back. After liposuction, your back probably will not have enough loose skin to remove with a relatively large surgery and additional risk.

It is my experience that patients who have lost near 100 pounds have enough laxity of the skin circumferentially around their body that the only way to adequately contour their abdomen, flanks, back and buttocks is a 360 tummy tuck. Your extended tummy tuck would go well past the midline of your flank onto your back and the small extension of the incision across your back would maximize your contour with no additional increase in your recovery time, etc. Meet with a surgeon who performs a 360 tummy tuck to get a clear discussion of all your best options.

based on your photos, you look like a good candidate for a TT. Full tummy tuck should tighten your abdominal muscles and remove the excess skin. Flanks liposuction would give you a better shape. I would suggest you to consult a board-certified plastic surgeon because only after a thorough examination you will get more information and recommendations.

I would NOT pursue 360 skin resection, a standard tummy tuck skin resection will suffice. Liposuction of your hips and waist will take care of the rest of your mid-section very nicely. Something else to consider . . . if you DO NOT have noticeable diastasis, meaning a bulge in-between your six-pack muscles when you do a sit up or plank type of action. Your muscles look flat in your photo so this is likely not a major issue in decision making. Assuming so, the I would recommend liposuction with skin excision and umbilical transposition. This is a tummy tuck type of skin excision excluding the muscle tightening. This can be done with an oral cocktail for sedation and local tumescent anesthesia, so you never go "under". There are no drains with resumption of activity as tolerated. There is an overall lower complication rate and down time compared to a traditional tummy tuck. This is a great option for many women out there . . An in-person exam by an experienced body plastic surgeon will help guide you in your decision. See attached reference for reference. While it is difficult to tell from the photographs alone, you would likely benefit from 360 liposuction in addition to an abdominoplasty. However, this recommendation could change with additional weight loss. It is important to achieve a stable weight prior to any body contouring surgery. I typically recommend that patients maintain a stable weight for at least 3 to 6 months prior to surgery. This assures that the contour achieved with surgery is more predictable and long lasting.
