It's hard to fully evaluate your current anatomy due to the limited views in the photos your posted. Given your description, the best way to "lower" the nipple areolar complex is to raise the inframammary fold. This would shorten the distance from the nipple to the lower fold of the breast and therefore improve the relationship between the lower and upper parts of the breast. The previously mentioned approach wouldn't change the lack of fullness in the upper part of the breast. Volume enhancement could be achieved with breast fat transfer or breast implant. We perform breast lifts, breast reductions, and breast fat transfer with the patient's comfortably awake in our office. No general anesthesia required.

Yes, your breast can be revised. Usually the nipple placement is correct, but your breasts sag below. Your breast can be elevated again and if needed, be made smaller. Smaller breasts are less likely to sag.

A revision can help to improve breast shape. You may be a candidate for auto augmentation as well. This approach uses your own tissue to create upper pole fullness without implants. This is not a fat transfer. I've included a video on this approach that I hope you find helpful.
