Dear Doctors, I have filler that needs to dissolved below my eyes. I read here before that ultrasound based dissolving can take place. But all the doctors I asked do not have ultrasound in their practice. So how can this be done? How do I find a doctor that can dissolve the filler and has an ultrasound machine? Thanks! K.S.
Answer: Using Ultrasound for assisting in dissolving fillers. Steven F. Weiner, MD There is a new trend with using Ultrasound to detect and help guide filler problems. Many docs haven't even heard of it. Ultrasound enables the doctor to see where the problem is, how much filler is there, and in some instances, guide the needle to the hyaluronidase. Dissolving can be much more accurate, instead of flooding the area with hyaluronidase. I will be training throughout the US on this type of management and evaluation. It is especially good for vascular occlusion, where blood flow is abnormal and in targeting the area of occlusion.

Using ultrasound for dissolves is extremely helpful as it allows the filler to be identified and with an experienced ultrasound user, allows real time guidance of the needle into the pocket of filler to be dissolved. In spite of this, I am not familiar with any literature that supports use of ultrasound to dissolve or assist in dissolving filler. The only thing I am familiar with is hyaluronidase, which works very well when used properly for the right indication. Probably the reason the doctors you asked don't have ultrasound is the above, there is no good reason to have it unless you actually do something else where it is useful, and, even then, most have someone outside the office do the ultrasound. Don't try to practice medicine, but, instead, trust an expert in the field. Not picking an expert in the first place, in fact, may be one of the reasons you need something dissolved now.
