Uneven nostrils - how can I fix this permanently? (Photos)

Question

I had a rhinoplasty in May 2023 to proportionately reduce the size of my nose. After the rhinoplasty my nose was wider than pre surgery, the tip is too high, my columella was still hanging and my breathing was compromised. After a year I had another small procedure with the doctor to fix the columella. My nostrils are now uneven my nose still feels large and not proportionate and my doctor is suggesting fillers to fix the nostrils which I don't want as I want this fixed permanently and evenly. I didn't hate my nose before but I hate it now and would appreciate some guidance on the least invasive way of fixing it without and grafts or complications if possible.

As far as I can see from your photos you have a hanging columella, I recommend revision rhinoplasty for a more natural and feminine appearance that is more compatible with your face, this will be a permanent solution for you.
It would be better face to face consultation.

Depending on your examination, microdroplet Silikon-1000 nasal injections may be considered for lowering your nostril margins, and for improving upon nostril margin asymmetry. If you are indeed a candidate, this would be the path of least resistance for a permanent Improvement. I hope this helps!

Hello and thank you for your question. Based on your photographs, you may benefit from a revision to improve your nostril symmetry. Maneuvers for this would include tip derotation and placement of extended alar contour grafts to help improve the nostril symmetry. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

The wide nostrils can be narrowed with alarplasty. However, symmetry is impossible. There can be some  improvement in symmetry but not significant. 
The columella is hanging and makes the tip look droopy. It can be pulled back with columellar reduction. 
Both procedures can be done under local at the same time.

A hanging columella is common after open rhinoplasty. It is simple to fix by trimming off the excess cartilage that is dragging the columella down. Alar reduction would also improve the result

Hi,Your right nostril apex seems higher than left one.This may be due to retraction of that side more than other side.It gives us more info to see before photos of each procedures.Sometimes nostrils are asymetrical from the beginning of the story but awareness and noticeability are not as much as after the procedures.In conclusion, harvesting ear lob composite graft(not only cartilage but also skin over it) may be an option for your deformity.Have a nice day,good luck.
