Twisted columella, uneven nostrils, deviated septum - do I need a third septorhinoplasty? or frourth surgery? (photos)

Uneven nostrils. Twisted columella. Deviated septum (again!). Want to correct due to self esteem & confidence. Surgery is the only solution. Surgeon reassuring, very experienced. Advises irradiated cartilage for further support. Also place betw nostrils to keep in place/avoid twisting. Open procedure. Skeptical/afraid of any issues as would be 3rd septorhino and 4th op overall. Don’t want to make it worse. Understand there are complications and limitations. Would like objective opinion.

The timing for revision rhinoplasty, even for a small bump, tends to be sometime from 6-12 months postoperative, depending on someone's examination. Good luck! Dr Joseph

The visible white dot on your nasal tip is likely an edge of your tip cartilage that is visible through your skin. Depending on your examination, a few droplets of Silikon-1000 may be considered for camouflage. Hope this helps you. Dr Joseph

The nose can be lengthened with cartilage grafting but unless your septum is untouched it Woukd be necessary to use Rob cartilage. Revision rhinoplasty is usually more complex than the initial surgery, and so more costly.
