Is it possible to have a lift and implant with minimal scaring? (Photo)

Question

I had a lift with galaflex and fat transfer my surgeon Didn’t bring the nipples in the center enough(he argues it can’t be done as my breasts are naturally sitting there and it can’t be changed. Now I want a perky round C cup and smaller nipples(I also have a bad tattoo on now). How can I achieve that with minimal scars most preferably a peri Areola lift? Some surgeons told me I would require a lollipop lift some say the full T redone some say Origami can be done but only at 2 stages no impant first as without lollipop it will open up! In terms of under/over/dual muscle my pinch test is 2cm they are not prepared to go under the muscle. I have very saggy naturally breast so I am afraid it will drop. I am only looking at tiny round implants of low projection 150-180 as a first implant taste. My Crysalix result even with a 150cc looks massive.

Hi there thank you for your question and sharing your photo. I understand that you are not satisfied with your current result, and you're looking for other recommendations to improve the appearance of your breast. On exam of the photo, it looks like your breast has bottomed out (the bottom fold of your breast has dropped lower) despite placement of the mesh. I agree that moving the nipple more towards the center would not be possible given the natural shape and position of your breast. 
A vertical lollipop lift may be more advantageous in this scenario, as it can help lift up the bottom fold, and therefore also raise up your breast/chest length. A small implant would be the most reliable way to add more volume in the upper chest. While the origami/autoaugmentation technique sounds tempting, the fact that your breast tissue has also started to fall despite the mesh also means that it will likely fall when the auto augmentation is performed. Over the muscle vs under the muscle would be mostly based on surgeon preference in this situation. Hope that helps!

It appears to me that your nipples are still too low. Your lift needs to be redone probably with anchor scars. Some surgeons might recommend lollipop scars but I don’t think that would work in your situation

Based on your photo and goals, achieving a perkier, rounder breast shape with improved nipple position is possible, but a peri-areolar lift alone is unlikely to provide enough lifting if you have significant skin laxity. A lollipop or anchor-pattern revision often offers more predictable, longer-lasting results. Small implants may enhance upper pole fullness, but the choice of lift should be based on your tissue quality rather than scar preference alone. A consultation with a plastic surgeon experienced in complex breast revision is recommended.

Your breast sit white on your chest wall and have significant divergence.
The divergence will be amplified with the projection of breast implants.
Your surgeon was correct when he said that this is how your breast are and they can’t be changed.
We cannot move the breast onyour chest wall and we cannot change the breast divergence or outward pointing angle.
Your nipple position is quite good and mostly you need upper volume, which is best achieved using an implant.
Your best option is to try to find the most experienced surgeon and let them do what theythink is best for you.
Finding the right plastic surgeon is far more difficult than most people realize.
I think your situation wants an in consultation and I don’t think you’re going to get meaningful information from posting on this website.
