Hi, Procedure done Aug22, bulbourous nose and wanted it smaller From a side view it looks a bit straighter, however it still curves upwards towards the nip. I was hoping for no curve. What I'm most concerned about is the front view. It doesnt look straight and still looks big, not at all symmetrical and lumpy. Also the septum (i think its called) isn't straight. The photos with blue background are the before photos. Do you think i need a revision? Are you seeing what im seeing?
Answer: I would wait longer Hello and thank you for your question. After rhinoplasty (nasal surgery), the nose can become REALLY swollen, and this swelling can take a LONG TIME to resolve (up to 1-2 years to fully resolve). Even though most of the swelling should resolve within the first 2-3 months after surgery- at 12 months postop there can still be some swelling present. I see what you're saying about the tip, but it is definitely better than the pre-operative photo, and some of this can still be attributable to postop swelling. From the frontal view, the asymmetry that you're noting, although present, isn't very substantial or immediately apparent. I constantly remind all my rhinoplasty patients how important it is to be patient after rhinoplasty surgery as it can take up to 2 years to see what the final nose will look like. I recommend that you continue to observe things for now until you are at least 2 years post-op, and stay in regular contact with your operative surgeon to help gauge expectations and discuss next steps. I hope this helps! Best, Dr. Donald Groves Plastic Surgeon

Answer: A revision could help Thank you for sharing. Certainly, agree with you regarding your assessment of your nose. A revision rhinoplasty would potentially allow to correct these issues. In particular, you would want to have the nose straighter and the lower portion of the nose decreased of width and projection.

Answer: Complex rhinoplasty Did you have a previous rhinoplasty or just one operation? Your preop pictures look like you’ve had a reduction rhinoplasty in the past. Based on those pictures, your procedure was fairly complex and appropriate expectations should have been set based on that. Building up dorsal volume is not straightforward and he usually requires some type of graft. Rhinoplasty is a fairly general term. Within the scope of that procedure are many different involved steps, depending on what the underlying problem is. It would be helpful if you could describe the exact steps involved in your rhinoplasty. It would also be very important to know if your rhinoplasty what is the secondary procedure or a primary procedure. Dorsal reduction is fairly straightforward while augmenting a previously reduce your summer is very complex. Based on the information presented, I think you deserve in person a second opinion consultation to get an accurate assessment, and better understanding. For proper second opinion, consultations come prepared bringing with you all before and after pictures and previous operative reports for any previous nasal operation.
