I plan to have a lip lift to correct my long Philtrum but I also have a protruding lower lip/ deep Mentolabial Sulcus which I would like to address first. I have read that chin implants may help but I would still feel that my lower lip droops/sticks out too much and shows too much of my bottom teeth when my mouth is at rest. What surgery could I have to correct this? (female, age 40)

Answer: Lip reshaping surgery Based on your photograph, an experienced plastic surgeon in lip reshaping would ensuite wether you had filler in your lower lip or not. Since you have a flaccid lower lip which is overly protruding to begin with, filler in that lip will weigh it down causing it to fall forward which will give the allusion your sulcus is deeper that in this. This flaccid lower lip is directly related to your recessed chin which does not provide the support of the lower lip that is needed. A chin implant will not only correct your chin recession but also if correctly placed will provide increased height to your chin and minimize your “double chin”. Care should be taken to insure that the vertical dimension of the chin implant is not too much otherwise it can make your fold deeper. Finally, with proper chin support and removal of the excess filler, a lower lip lift in the form of an I-T plasty will help. It should be remembered that filler can also help but it must be done utilizing a special vertical technique, which I have called Caisson Contouring. I hope this helps, best of luck !
