I think you should have that mole looked at and either biopsied or removed. The surgery or procedure to remover the mole can be quite straightforward as long as the lesion is benign. There are a few options. One would be to simply excise the lesion. This option would leave a vertical scar that would go from your lip to the skin just under the lip. I do not anticipate the skin scar to be very long at all and the scar on the lip would not be very noticeable either seeing as the lip heals very well after any sort of surgery or insult. Another option would entail removing the lesion with some surrounding skin and then performing an advancement flap where some of the inner mucosa of the lip would be brought up and over to cover the defect from the mole removal. Those are just some options but I would definitely start by having the mole examined by a dermatologist, primary care of plastic surgeon first and foremost especiaily if the lesion has changed recently.

Moles on lips can be removed using the same techniques as used to remove moles on the skin elsewhere. As you say the question is what the scar will look like and this is difficult to predict. Scarring varies from one person to another and from site on the body to another. I advise you to have a consultation with a qualified and experienced plastic surgeon who regularly deals with skin lesions so they can examin you and give an in person opinion. You could start by asking your GP for their advice.
