Hi all, The fact that my lower face is very disproportionate to my flat mid face has been bothering me for years now. I am 25 yrs old, BMI is 21.5. I'm not sure if I'd benefit from buccal fat removal as it seems so but when I was younger, my face actually looked less puffy - which leads me to believe it could be masseter hypertrophy. Please see the other procedures listed above that I'm considering (would prefer a long term solution!). What would you say is most advisable?

Answer: Depends on examination In person examination plays a big role in the answer here. You have a retrusive chin which would could be addressed with a chin implant. Based on your photos, the buccal fullness appears to be more anterior (which would be treated with buccal fat pad removal) rather than posterior (which would be treated with masseter botox). For the submental area, in person exam would be helpful to see if there's a little fat there for lipo, or if just radiofrequency skin tigthening will help. For the midface, fat grafting or filler can help add fullness and contour.

Answer: Genioplasty, chin lipo + buccal fat removal? Or masseter Botox/fat grafting/Morpheus8? Based on the series of nicely posted photos, I recommend NO buccal fat removal since the buccal areas already slightly indented. I would offer mini anterior facelift with SMAS plication to lift the jowls you are seeing, Radeisse injections to chin to in rove projection with chin lipo, masseter botox also. Fee $8,000.
