Hello @katiegreggs, thank you for your question.I suggest you consider both options, because Botox applied on the masseters is not the same as a Bichectomy. Botox is applied to the masseter so that the masseter loses strength and the lower jaw line does not appear as thick, and a Bichectomy is performed to reduce chubby cheeks, as it removes the fat from the inner pannicle of the cheek. Seeing the 1 frontal posted photo I do not see buccal fat. I see jowling with laxity of mid face and cheek/malar hypoplasia, along with temple indenting. Best is virtual consult with...... Offering options of 1-stitch FaceLift, HA fillers to temples, cheeks, threading with newer PDO threads, or combination of all the previous mentioned options.
You can consider buccal fat reduction. It will help with lower face fullness. However, it won't prevent jowling down the line since those two are not related.
