As everyone ages there is a loss of volume in the midface, the jawline isn’t straight and defined due to the jowls and a fullness can develop underneath the chin, which is a combination of some loose skin, relaxed muscles and fat, creating an obtuse neck angle, which is the opposite of a youthful neck contour, which is closer to a 90° angle. To fully correct all of your concerns, I would recommend an extended SMAS flap, which would smooth your jowl, straighten your jawline and help improve the redundancy of the muscle underneath your chin. In addition to that, for the best result, I believe you would require a small incision just under your chin so that some of the extra fat in that area could be removed to deepen your neck angle as well as approximate the loose muscles of your neck to give you a better neck angle. This should give you the most complete facial rejuvenation. This procedure is usually done under conscious sedation plus local anesthesia, and most patients can go home that afternoon or you may wish to opt for one overnight stay in the facility for monitoring.
