Not enough breast tissue for uplift after implant removal - what are my options?

Once the implants are out- and healing occurs- your breasts will most likely return to the pre-op condition similar to the posted pre- surgery photo.

If the picture on the left are your preoperative photographs, in my opinion, there is enough tissue for a breast lift. I would suggest an anchor mastopexy. If you want more fullness, fat grafting could also be performed. You must be able to accept the scars around the areola and vertically down the middle of your breast.

In your before photograph, your breasts were low on the chest wall and you did have some breast tissue. At this time, I recommend explantation and mini lift. Using a circumareola incision, the implants are removed, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. Later, fat transfers can be performed if additional volume is desired.

An option for women who want implants removed and not replaced is to replace some volume with fat transfer to the breast. Fat transfer to the breast in these cases is often combined with a breast lift to tighten loose skin and improve nipple position,

Thank you for your question. After removal of your implants, you will have excess skin that would benefit from tightening. There there will be enough tissue for a small left, especially because your skin has been stretched out, and some of that tissue can be reused for the lift. However, you will be dramatically smaller. It does not appear that you have enough fat to undergo a fat transfer. Your other option is to place a significantly smaller implant partially under your muscle to give more volume but I’m more muted augmented look. Good luck.

You could have the removal and the minimally invasive radiofrequency skin tightening like Renuvion or ThermiTight to give a safe lifting.

I would recommend removal of your implants and auto augmentation with your own breast tissue along with a breast lift. After having large implants your skin will sag and you will be left with extra skin compared to your pre-op photo. This means you are a candidate for a breast lift. When performing a lift there needs to be enough breast tissue present to form a nice breast shape. Without that you could consider a smaller, low profile implant or fat grafting. From your photos you appear to be thin framed and may not be a good candidate for fat transfer.
