Hello. I have had breast implants for the last 15 years. They are 325cc, silicone, and placed under the muscle. Recently I found out the right breast has a “small” intracapsular rupture. I’m a young 45 year old. I do not smoke, drink, use tanning beds etc.. the surgeron that has seen me for a consult said he can do a breast lift, he also said that it’s up to me. more, he said that I do have good skin. The surgeon would not give me his opinion if I really should have the breast lift. Also, I’m no
Answer: Considering options Based on your photos it appears you would have the best result from a breast lift when you have your implants removed. This would raise the position of the nipple as well as remove any excess skin. As you have a rupture it is recommended to have both the implants and capsules removed.

Answer: Explantation Hi Compassionate5010, Based on your photos, I think that removing the implants ONLY without a lift, would leave you with an unappealing shape to your breasts. It seems that most of your breast tissue has migrated towards the bottom. I would recommend that you also have a breast lift to reposition the breast tissue. Good luck!

Answer: Breast lift Dear Compassionate5010, my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. Answer: Breast lift with explant? Hello @Compassionate5010, thank you for your question. You could have an explant with total capsulectomy and given that you have good mammary tissue I consider a breast lift to reacomodate the mammary tissue and lift the areolas and nipples with BRALAN technique. The breast shape is improved by reorganizing the breast parenchyma. To shed more light, I share a video. If you are considering a breast explantation, consultation with a plastic surgeon who is experienced in this procedure is essential to ensure optimal results. Best wishes! Alan Gonzalez MD, FACS. Answer: Explant with Bellesoma Method The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.
