Hi, I had a breast reduction 8 years ago due to pendulous breasts. Now they have bottomed out and are flat at the top . I’m booked in for implants . However could the surgeon take the tissue from the bottom of the breasts and resculpt them or would implants be better . Thanks
Answer: Revision breast surgery Hello, Thank you for the question. Sometimes, extra breast tissue on the bottom can be removed to make the breasts smaller and less bottomed out, or breast lift can be performed to create a more youthful and lifted breast shape. Adding breast implants will certainly increase upper pole breast volume and give you a fuller cleavage up top. I would encourage you to seek a consultation with a Board-Certified Plastic Surgeon to further discuss your options, specific techniques, concerns and expectations. I hope this helps.Best regards, Dr. Karanetz Dr. Irena KaranetzBreast Fellowship-TrainedBoard Certified Plastic Surgeon

Answer: Implant vs lift HelloThank you for the question. Your breast are asymmetric with lack of tissue mainly on the upper part, a good result can be achieved with breast lift with or without implant. The decision depends on the desired look and size.

Answer: Lift Yes, the lift can be redone to help reposition and tighten your breasts and remove excess skin. Small implants could be added under the muscle for better upper breast fullness if you want.

Answer: Bellesoma Method The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.

Answer: Revision surgery Dear Luminous9504, breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery. Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size. While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals.
