You would need a lift/mastopexy. You have too much breast on your abdomen. An implant will provide more fullness and shape but the breast would "waterfall" over the implant and not look pretty.

You should have an annual pcp exam and report all medications and health/systemic issues to your surgeon. He/she may require blood work, ekg, medical clearance based on your health history.

If you loose 15-20% of your body weight after surgery it will have an impact on your results. Its best to be at an ideal weight for the best results.

It appears you have lack of elasticity of the chin and neck. A necklift would remove the excess skin giving you a more defined jaw line and chin. A in person consultation would decide if this is the best procedure or possibly lipo suction if there is fat in that area upon examination,

I will sometimes do both upper lids and brow together. The quickest recovery and least expensive would be to just do the upper lids. You could try botox to lift your brow as well.

With a tummy tuck I usually always lipo the mons. During this procedure you get a big lift in this area as well. So this combination will resolve this puffy mons area.

A vertical breast lift will make your areolar complex smaller, matching and lifted. You will look perky and cute, small breast.

You might have to large of a width implant. Maybe one that is more narrow, more projection and firm(cohesive). Going down in size will help as well, but it depends on the look you want.

Usually a muffin top is your lower back/hip area. This area can look like a "muffin top" after surgery because an abdominoplasty pulls everything in front so very tight and if you have pinch able fat in the lower back/hip area this will looks more emphasized naturally. I like to address this with liposuction in congruent with my abdominoplasties. If you are talking about a "fat pad" above the pubic area you will want to ask your surgeon if they Lipo suctioned this area. I always address this area as part of the procedure.
