Is a repeat of the fat transfer an option? Or unlikely since it failed before? (photos)

First, I'd like to congratulate you on your good surgical outcome. I think there's significant improvement despite losing some fat graft volume 6 months out.  It's evident from your recent photo that the fat graft did take and it didn't fail completely.  Often times, patients will need multiple fat grafting procedures to attain their desired volume.  And despite overgrafting, there's only so much fat that would survive each time due to the limited blood supply. Meticulous surgical technique and how the fat is processed may affect the results.  You should consider consulting with your surgeon again for another round of fat transfer. Best wishes.  

Simple answer to your question is yes. You can have repeated fat grafting to your face. It seems at six months period your swelling has subsided and now you're seeing the actual results. A second fat grafting would be a good option for you.

Fat transfer doesn't always stay fully, post treatment we inject PRP/Sculptra to continue long term viability.  However, studies have shown that repeated injections help maintain the volume, so another treatment would be good, however you can use other fillers at this time and maintain your skin type and tightening with non invasive options like lasers, microneedling and radiofrequency or ultrasound.  Please see an expert.  Best, Dr. Emer.

The result of your lift looks great, and you appear to have more fullness in the face due to the fat transfer. Although you might have been seeking more dramatic results, your results look natural and it does not look like the fat graft "failed". The possibility of additional fat graft to the face is probably viable, but the final say is up to your surgeon. Temporary fillers are also an option, but fat grafting will be much more lasting. If you are 6 months out and not happy with having enough, you can re visit your PS for another procedure. Some areas of our face or body can absorb fat faster than others.  I tend to over fill when I transfer fat due to the fact that the body does re-absorb approximately 30 % of it. 

I think in your case it may be difficult to make an accurate assessment in regards to what percentage of grafted fat actually survived long-term.It's unlikely that all of the fat failed.it can be difficult to distinguish what represents swelling and what represents fat volume enhancement when multiple procedures are done at the same time.Your before and after pictures look nice.Everybody looks their best three months after this type of the procedure.A lot of this has to do with swelling which makes fine wrinkles go away and enhances facial volume.I think your brow lift shows excellent results at six months.It looks natural and like it was done very well.without knowing the volumes of fat that were drafted or what the intention of your plastic surgeon was it's hard to say if you're fat transfer procedure was a failure or simply not an aggressive attempt of restoring facial volume.having operated look is in my opinion highly undesirable outcome so staying on the conservative side is generally a good idea especially when it comes to restoring facial volume.I suggest you follow up with your plastic surgeon and share your concerns with him or her.During consultations in regards to facial fat transfer I usually discuss the possibility of needing to have fat transfer done more than once.If done a second time it's generally much more limited and can be quite easily done in an office setting.A couple of things come to mind.One is the idea of using temporary fillers to try to establish the most desirable volume restoration and where the volume should be placed prior to any possible second procedure should you ever contemplate having this done twice.Please do not take offense if I offer unsolicited recommendations.I think you would look phenomenal if you used a small amount of Botox and filler for your frown lines.You would also look really great with a tiny bit of filler in your nasolabial fold and possible some "conservative"lip volume.perhaps you were seeking a more dramatic change.Your results in my opinion looks really good it's natural and that's a much better place to be and then having been over treated or something having gone awry.Best,Mats Hagstrom M.D.
