I do a significant amount of calve and ankle contouring and put fat into calves all the time. Its very very rare for someone to need ankle fat and most often they need it removed. I suggest seeing an expert in leg contouring and get a formal evaluation. Bring in pictures of what you are looking for. Best, Dr. Emer.
I appreciate your question.Yes it can. However, the likelihood of the fat transfer taking is unpredictable. The main reason for this is the lack of tissue to graft fat into. For grafted fat to survive it has to be surrounded by vascular tissue. The ankles have very little vascular tissue to graft into since they're mostly made up of skin, bones and tendons etc. Fat is generally successfully grafted into fatty tissues or muscle. A fat transfer should help to replace some volume there. You will have some scar tissue, so may need to have it done more than once depending on the amount of fat that survives. The additional volume will help with the wrinkling but may also need to continue with the botox there.
