Deviated Septum + Concave down nose + age - Should this be fixed together in one surgery? (Photos)

Question

male, 18, 5'10", 140lbs; I think it's pretty clear that the midline of my face is skewed because of my nose, worsened by the fact that when I had braces, my ortho matched the midline of teeth to the tip of my nose rather than the bridge. From the minimal research I've done, this is called a deviated and would require a septoplasty. However, I figured that since I was eventually going to go through the trouble of getting surgery, i might as well change the concavity of my nose from up to down with a rhinoplasty to achieve a more "button nose' effect. Whenever I bring up the subject to my family they always bring up the fact that I am young and the issue might drive itself away and/or need additional recovery time, neglecting the fact that when i am in college I will have all the less time. If you have any thoughts on the matter it would be a great help! (sorry for writing an essay but I have the AP lang exam on wednesday and this made could practice for my tone)

It is completely understandable that you’re noticing the "skewed" effect on your facial midline, especially since dental work can sometimes inadvertently emphasize a nasal deviation when the teeth are aligned to an off-center tip. At 18, your facial growth is largely complete, so your family’s hope that the issue will "drive itself away" is unfortunately not how nasal cartilage and bone structure work; a deviated septum or crooked bridge requires a surgical septorhinoplasty to truly straighten. While your desire for a "button nose" effect (upward concavity) is a common request, I would caution you to prioritize a strong, masculine bridge and a straight midline first, as over-rotating a male nose can sometimes look "surgical" rather than natural. Since you’re heading to college, your timing is actually quite practical, as the primary recovery is only about two weeks; just keep in mind that a "button nose" look involves thinning the tip and lowering the bridge, so you’ll want to find a surgeon who specializes in ethnic rhinoplasty to ensure they preserve your unique character while fixing the symmetry that bothers you.

You are a candidate for a rhinoplasty and septoplasty.  Age will not improve this.  Your nose is hanging downward and the dorsum is crooked.

Hello and thank you for your question. Based on your photographs, you are a great candidate for rhinoplasty. Maneuvers for your surgery would include dorsal hump reduction, tip refinement, nasal tip deprojection, tip elevation, correction of deviation, among other maneuvers. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
