Thank you for your question and picture. There are several options for treatment of the neck, however in your situation I would expect that surgery would give the best degree of improvement. A combination of liposuction and a neck lift will reduce the fullness and the loose skin. The more difficult issue are what are commonly called neckless lines. These can be treated with intra-dermal fat injection with significant improvement. I would suggest you consult with a plastic surgeon with a lot of experience in neck enhancement surgery. Good luck.

The kind of lines you have on your neck, the kind of go from side to side as opposed to up and down, are the very hardest to treat. They are called "necklace lines". There is no wonderful solution to treat these that is 100% reliable. A traditional neck lift will not work. Some forms of treatment can include internal release of those attachments, fat grafting, fillers, laser tightening devices, etc. Most likely, none will achieve a very smooth, lying free neck. Neck is one of the hardest areas to treat. Unfortunately there is no good for improved treatment. The best option remains to be Thermitight treatment.

Everyone has some degree of neck rolls, some more than others. The creases are caused by attachment of the skin to the muscle underneath. The roll portion is where the fat under the skin is thicker than where the creases are. The attached areas must be released and fat can be put under to try and prevent reattachment. In spite of these efforts the rolls may return. See a board certified plastic surgeon for a detailed consult about your options.
After looking at your photos you have some hypertorphic scars that may need soem sstreoid injections or some topical steroid creams.these will go down in time.
