Fillers such as Juvederm Voluma XC are a great option for flat cheeks. Juvederm Voluma XC is the first and only HA fillers FDA approved for Midface lifting. Even though the injections are in the mid face, studies show that patients see the results in their nasolabial lines and tear trough areas as well. However, if more correction is needed, additional injections with Juvederm Ultra or Juvederm Ultra Plus XC is also a good choice for the nasolabial folds. One must balance the amount of filler with the desired correction to maintain a natural look so the patient does not appear "over filled."Good Luck.
I recommend fillers to soften the nasolabial folds themselves and also lift around the mouth and cheeks to build underlying anatomical support. In our practice we combine Sculptra with other fillers "layered" on top and lasers, skin tightening treatments such as Thermage, Fractora, and Renuvion (J Plasma), thread lifting, and peels to get the most conservative, natural look. Thank you for the great question and the photo. An in-person exam of your facial structure and elasticity is needed to make the best recommendation, but fillers and cheek implants are great options. For fillers, I use Radiesse or Voluma in this area. These fillers are longer-lasting and specifically designed for volumization. They are placed deeper, along the bone, and lift the soft tissues to give a more youthful, full face. Cheek implants do the same thing, but are permanent (i.e., don't require repeated injections). These are more cost-effective over time, and may be removed at a later date if you decide to do so. A final option is a midface lift, with or without a lower eyelid lift. This treatment has the most to offer, but also has the longest recovery time and largest expense. Please visit with a couple board-certified surgeons before making your decision. Best of luck!
Voluma is FDA approved for mid-face filling. It is the first filler approved for this specific purpose. As a matter of fact, in the FDA study, a significant number of patients that had Voluma in only the cheeks felt their Nasolabial folds and even under-eyes looked better. I often combine Voluma in the cheeks with Juvederm in the nasolabial folds as well.

In most cases, the procedure may be completed in no more than twenty minutes with immediate return to work or social activities. Nonsurgical augmentation with volumizing fillers has the additional benefit of being able to be modified in accordance with age-related changes in the area itself and surrounding regions. From the photo provided, it would seem that nonsurgical cheek augmentation would provide the necessary definition and lift to this region. A secondary benefit that is often seen following nonsurgical cheek augmentation with volumizing fillers is improvement in the smile lines (nasolabial folds) as a result of the lift achieved in treating the cheeks.A good way to determine whether someone might benefit from nonsurgically revolumizing the cheeks and the mid-face region is to ask them to smile. If the smile lifts the fallen tissue back into place and rounds the contours of the cheeks making them appear more youthful, it is a good indication that nonsurgically volumizing the above regions with injectable volumizing agents, would yield immediate, durable, and gratifying results.For nonsurgically augmenting cheeks, I prefer to use Radiesse or Radiesse combined with Voluma or Perlane L. I rely on these agents for these purposes in my Upper East Side New York City practice. In Israel, where I have a satellite office and where a far greater number of more robust, regulatory agency approved volumizing fillers are available, I prefer to use Stylage XXL, either alone or in combination with Radiesse or Restylane SubQ..It is crucial to seek treatment by a board certified aesthetic physician with extensive experience in treating these areas.

The 'mage you have provided demonstrates areas of midface soft tissue atrophy (hollowness) and some soft tissue excess (fullness) that are immediately adjacent, and these changes represent a very typical pattern of midfacial aging. The areas of fat atrophy and excess fatty fullness need to be managed together but in very different ways to produce a youthful and natural-appearing surgical result.The 'before' photo shows fat atrophy at the 'tear tough' which extends obliquely across the entire lower lid / cheek junction. This demarcation between lower lid and cheek due to fat atrophy (and gradual atrophy of the underlying facial bones as well) generally starts in a patient's 30s and is often very obvious by the time one reaches their 40's or 50's.A simultaneous, adjacent change with aging is an increase in fat volume in the anterior lower cheek, immediately above the nasolabial folds. This creates a heavy appearance in the lower cheek and deepens the nasolabial folds. The increase in anterior lower cheek fullness creates a situation where no amount of fat grafting in the nasolabial folds will actually improve the appearance of the nasolabial folds, unless some of the excess fat is simultaneously removed from the area immediately above. If one tries to improve this by fat grafting alone, then you unfortunately end up with a big lump at and above the nasolabial folds.You have early signs of aging and an attractive face. The right procedure could do wonders. The wrong procedure could age the face. I would strongly advise against cheek implants. The photo makes it difficult to know exactly what would be best for you. take some time looking at before and after images. When you find plastic surgeons with many before and after images that look like results you'd like, take time researching those doctors. You then could schedule in person consultations, or phone consultations if you were to provide many more images from different angles.
