From your photo though it appears that you need a breast reduction. Depending on your surgical goals, the placement of at least a small implant could be of benefit to you as well. Based on your narrative, a lift/reduction seems to be the best surgical treatment, as it provides a lift to your breast tissue, removes excess skin, and places your nipple in a higher, more youthful position. It looks like an anchor type incision would be best, as this is the most powerful, shape-changing lift. Hope that this helps.

After having analyzed all the information and photos provided to us, I realize that you have asymmetrical and sagging breasts.In this regard, i recommend a breast lifting with implants to get a bigger breast with adecuate projection. If you are happy with the overall volume, then you can probably just have a lift. As for implants if you desire to go bigger, then you need to be evaluated in person.
Thank you for your question. Without knowing what your goal is, it is impossible to give any meaningful advice. Given that your starting breast size is 34G, you may not need an implant. Sometimes, a lift alone will give you the size and shape you're looking for. The best thing to do is to see a board-certified plastic surgeon in person. They can interview and examine you and then give you meaningful advice.
Certainly from the photo, you need the equivalent of a reduction with the vertical T incision to address the degree of sagging in the breasts and then an implant to create upper pole cleavage if desired. Depending on the situation, nipples may need to be removed and replaced during the procedure. See an experienced, board certified plastic surgeon to discuss your options.

Some of my lift patients do not require an implant as they have enough volume. The type of implant, if used, is best determined by your PS. I prefer the Vertical technique, and not the " anchor scar" in the older technique.
