Occasionally, a small dog ear can remain after upper lid blepharoplasty. It is usually easily excised with a better resulting scar. The chance that it could be worse is minimal in expert hands.

This should not be something complicated to remove and a short office procedure.

The bump looks like it could be a small cyst under the skin rather than a true dog ear, which is excess skin. Either way, it should be able to be revised without making things worse.

You would be wise to consult with an experienced, board certified cosmetic dermatologist. Scalpel Sculpting, which removes the protruding surface and leaves the treated area flush with the surrounding normal skin, is one method for dealing with this. A second method would be to use low intensity electrocautery to flatten the bump so that the treated area becomes flush with the surrounding skin.
