Another possible reason for the appearance of a wide face is prominent masseter muscles, which are the chewing muscles at the angle of the jaw. These muscles can be especially large in patients who chew alot of gum, or grind their teeth at night. The size of these muscles can be reduced quite significantly with botox injections. These injections typically last 4-6 months before needing to be repeated, and there is some recent evidence that after about 10 injections, the muscle stays permanently smaller. Similar to what Dr. Yoo mentioned below, the angle of the jaw can be reduced to narrow the lower face. A heart-shaped face is youthful, and so I would typically prefer to narrow the lower face not the upper face. In the end, the site of narrowing depends on a patient's specific facial shape and goals. Both jaw-narrowing surgery and botox injections to the chewing muscles are probably most common in the Asian community.
