It's important to me that I have all overhanging skin removed. I had lipo of the abdomen, chest, and flanks about 20 yers ago, then gained quite a bit of weight. I've had the weight off (125 pounds) for 6 years now, and am left with a lots of skin laxity. Current weight is 174 lbs. at 5'10" tall.

Either of those options are fine. If you go 270 degrees you will probably need to be turned facedown fir part of the surgery. At that point why not go all the way around.

An extended Abdominoplasty / dermolipectomy (270 degree) is what you need to remove the excess skin of the entire mid-section. This approach will elevate the anterior & lateral thigh skin but not to the extend of a lower body lift. A lower body lift is a circumferential ie 360 degree dermolipectomy (surgical removal of skin & subcutaneous tissue) which is performed in such a way as to produce a much greater lift of the skin of the anterior & lateral thigh region , hips and to elevate the buttock tissues. The circumferential waist is thoroughly addressed with this surgical procedure as well. Technically speaking, Abdominoplasty suture repairs lax abdominal wall, dermolipectomy addresses excess skin only, the abdominal wall itself is not sutured. Many patients who I have performed circumferential lower body lifts combined with high volume liposuction do very well and look great without the extra time, cost, post operative pain & associated post operative complications which accompany abdominal wall suture repair. This applies to extended ,ie 270 degree dermolipectomy /“Abdominoplasty” as well. The thinning of subcutaneous fat and smoothing of skin skin which occurs with high volume liposuction off sets any gain which occurs in abdominal wall contour with suture repair of the abdominal wall. My opinion from my personal experience performing dozens of Lowe body lifts both with and with out 1) suture repair of the abdominal wall 2) high volume liposuction.Best of luck.

You have excess skin that extends posteriorly. It is hard to assess how far posteriorly because the underwear is distorting the skin envelope. You would be better off with a 360 body lift as that will give you more pull and pull in the sides better. A 270 tummy tuck may be possible but I recommend the 360 body lift.

Congratulations on your weight loss!  Since the excess skin is loose, the only way to effectively make your body contour tighter is to remove this skin.  The more you remove, the tighter the outcome.  An extended (270) abdominoplasty would do very well since there isn’t as much loose skin in the middle of your back, but 360 bodylift would make that area tighter too.  I would recommend that you do the 360 for the tightest result.  Best of luck!

Thank you for your question and photos. Your BMI is 25 and you  are a good candidate for body contouring surgery. Unfortunately, the posted photos do hide the areas of the lower abdomen and back for a good evaluation. With a 125 lbs. weight loss, it is most likely that a full abdominoplasty with a lower body lift will give you the best results. You also need liposuction of your back, chest and lateral chest wall at the same time.

based on your photos, you look like a good candidate for a TT. Full tummy tuck should tighten your abdominal muscles and remove the excess skin. Flanks liposuction would give you a better shape. I would suggest you to consult a board-certified plastic surgeon because only after a thorough examination you will get more information and recommendations.
