I suffer from a deep tear though (deformity) and as a result look always tired with visible eyebags. Right side is worse then left side. I was told a "normal" bleph couldn't adress the underlying issue. A Lower lid blepharoplasty would address the lid-cheek junction and tear-trough area. (So midface will be partly lifted to shorten the lower lid lenght and excess skin will be removed). I am 28 years old/young. Thank you for answering to my concern!
Answer: Transconjunctival blepharoplasty A transconjunctival blepharoplasty would remove the herniated fat that is causing the bags. Additional fat grafting along the tear trough and orbitomalar groove would further improve this.

Answer: Lower Blepharoplasty Common reasons for lower blepharoplasty include tear trough deformity or pseudoherniation of the orbital fat. An in-person exam would provide a more accurate diagnosis, but preliminarily based on your photos it does appear you have pseudoherniation of fat creating "eye bags.” They should also be more prominent when you look up. Transconjunctival lower blepharoplasty with fat repositioning will provide nice results and a better lid-cheek transition in this case. @drdonyoo

Correction of the orbicularis muscle/ligament attachments and redistribution of fat pads, should be all what is needed; it is all achieved using small incision placed inside the eyelid. In person consultation with a board certified plastic or oculoplastic surgeon, is recommended. Good luck.

Answer: Treating Deep Tear Trough in 28 Y/O I would advise against any blepharoplasty or mid-face lift. Having done these for years, I eventually gave them up as too invasive with too many problems and not as good of results as desired. Not even trans conjunctival fat repositioning gavotte desired results. I also used fat grafting with excellent results but have switched to Juvederm Voluma most of the time. It provides excellent results and lasts significantly long, up to and over 5 years. Make sure you see an expert as most injectors inject superficially and use a light filler, both brought with problems
