Best permanent treatment for hollow upper eyelids? Would structural grafting/fat transfer help? (photos)

I see three to four patients each week who have had fat grafted into their eyelids. While it is theoretically possible to soften the subrow area with grafted fat, grafted fat is like injecting scar tissue. It does not flow at all in a structure where it is critical that everything needs to slide. The effect is generally terribly unsatisfactory. Do not get talked into this. Generally fillers are no better. Your issue is that your anterior levator tendon is slipping in the eyelid cause the upper eyelid crease to retract upward into the anterior orbit. It would be helpful to have photographs of you before your facial surgery to understand what upper eyelid volume you had before surgery. In many cases, it is possible to reconstruct the upper eyelid anterior levator aponeurosis which is slipping and advance anterior orbital fat with the repaired tendon to help restore the appearance of an upper eyelid fold. There is no substitute for a detailed in person assessment.
