Does my filler in the eye area look misplaced? (Photo)

Question

Got my yearly filler n this time i had dips in the outer corners of my eyes that were really deep, Does the filler Look misplaced or is it just swelling? It seemed to be fine the day after and Then got a bruise and now it seems to be yellow and blueish tinted and seems to be a lump. I just had it done last Thursday, is this normal or not? The injector used juvaderm because restalyne never takes to tear troughs n never had a problem with Tyndall effect or with lumps in the area.

Juvederm is not ideal in the tear trough area as it does result in significant swelling. Injections in the tear trough area should be on bone and of small volume. Please go back to your doc and have hyalase injected to dissolve the filler.

Thanks for your question, At this point it is hard to tell , if it is swelling or your results, swelling can last up to 10 days to 2 weeks. Give it time to settle down, If you are unhappy with the results, it can always be removed with hyaluronidase. Follow up with your injector or doctor.

You most likely have swelling which can last 1-2 weeks after your injections. If you're unhappy with your final results, you can have it reversed with hyaluronidase and try a different technique next time. I recommend injections with cannulas to avoid complications. Best, Dr. Emer

Thank you for your question and for sharing photographs. Unfortunately, Juvederm is not the appropriate filler for the tear trough area. It will result in persistent swelling under the eyes because of the high water binding capacity. However, this can be resolved by using an enzyme known as hyaluronidase to help dissolve the filler. I recommend you see an expert and if ever you want to get your tear trough re-injected, Restylane would be the best option. Good Luck!

Bruises may happen often after filler injections and may take few weeks to completely dissolve. Tear trough injections may appear as swelling if filler is placed superficially. Follow up with your doctor is advisable.

The honest answer is that it is hard to tell at this point. Some swelling and bruising are common in the undereye area after filler injection given how delicate the tissue is in this area. I would give it 1-2 weeks and if things don't improve, I would followup with your injector. If the filler was placed too superficially then Tyndall could be a problem. Juvederm does tend to swell more in this area then Restylane. Either way, the filer could be reversed with hyaluronidase if you have any problems once things have settled in.

Thanks for your question and we always recommend that you have these injections in the offices of a board certified dermatologist or plastic surgeon as we are the true experts in the anatomy of the area and also how best to inject.  It does appear from your photos that the injections on your right side may have been placed too superficially and you have what we call the Tyndall effect.  You can do two things – one is to wait two weeks to see what everything looks like settled in or go back and get hyaluronidase injected now and start all over – that is what we would do if we saw this in the office.

Thanks for the great question. It looks like you may have gotten a bruise which is common after injection around the eyes. It is important to give the filler 2 weeks to settle and for the bruise to completely go away before an accurate evaluation can be done. I suspect that it will settle and look good and symmetrical at the 2 week mark. If it does not you should see your doctor and he/she can dissolve some of the filler if needed.

The tear troughs are a tricky area and many things could happen if done incorrectly. First, I am not a big supporter of Juvederm under the eyelids. You might get lucky and have great results but at some point an injection might not work because Juviderm is, in my opinion, too thick for the thinnest skin in the body. Looks like you might have had the product migrate and cause that appearance. IF it doesnt resolve in next few days with massage and time, you might want to have it reversed with an enzyme and try a thinner filler like Belotero which does wonders for the tear trough, and because of its intrinsic properties, has less of a chance to result in a tyndall effect. Please ask you injector. I use to practice in Detroit area and can recommend some good injectors if you like.
