Question

Previous set was 325 under muscle. Issue with breast corrected but after two kids, breast feeding and now much older they lay differently and feel more weighted and uncomfortable to me plus issue with left breast returned. After many good years with them. Under wire to blame? I think so. Thinking of explant but was 19 with initial breast surgery. Was. 32AA. Currently a 36 C. What to do? Afraid of outcome.

It's difficult to tell from the provided photo, but you may have a double bubble.  You can elect to have the implants removed at any time.  Because you have had them for 28 years the tissue will likely be stretched out.  You can have a lift at the same time as explant to improve breast shape.  You can also consider differently sized implants if you are still interested in more volume.  The first step is to visit with a board certified plastic surgeon near you for a consultation.

I assume from your question that you had the 325 cc implants replaced with 450's, and now have the indentation on the bottom of the breast. Possible causes are:Lowering of the fold to accommodate the larger implants. This may be best corrected with a lift.Release of the pectoral muscle. This is an overlooked cause of double bubble, because the muscle adheres to the capsule. It typically animates or pulls up when you flex. Consider converting to the split muscle plane to correct that.Scar tissue. This may be correctable with a release.

I can't really tell in the photo, but with such old implants this might represent scar tissue and the old implants probably warrant exchange.
