Dr. Black performed my breast ex-plant surgery. I cannot say enough good things about her & her staff & the surgery center where she performs her surgeries. ALL TOP NOTCH! I went in because my implants were hard & encapsulated. I was having some breast pain & had rashes that would come & go in my breast & abdomen area. I had a few other symptoms but really wasn't sure if they were related to my implants. My main issue was the extreme hardness & pain. She thoroughly examined me & decided I needed both capsules removed & sent to pathology for review after removal. I was on board with this decision & was more than confident she would do the best job out of any other surgeons I had looked into. After ex-plant & full capsulectomy my pathology came back with a rare cancer found in the capsule that surrounded one of my implants. Before my follow up appt. Dr. Black had contacted a physician at MD Anderson who is an expert in the type of cancer I was diagnosed with. She put me in touch with him & I went to see him 2 weeks later. I am happy to report I am cancer free but if it were not for Dr. Black removing the entire capsule I don't know what my outcome would have been. She did such an amazing job meticulously ensuring everything was removed. I am forever grateful for her skills & I highly recommend her as a surgeon. I had consultations with a handful of plastic surgeons about removing my breast implants during the ten years I had them, and every surgeon dismissed my concerns — except Dr. Black. I’ve been unhappy with my body for so many years, but when I entered my fifties things really took a turn for the worse. I’m a wife, mom of 2, grand mom of 3, am 52, 5’1”, 120 pounds and I’m very physically active. I hit the gym 3-5 times per week - mostly cardio - and of course none of that work could fix my low lying breasts or the loose, stretch marked skin on my flabby looking stomach. As we ladies do, there have been conversations between friends about wish lists. You know, if I could have anything done what would it be? Dr. Eileen Summer Black’s name came up quite a few times, as some of my friends had already had their wish lists fulfilled, and Dr. Black seemed to be well loved by all. I decided to go for it. I spoke to my wonderful husband, who by the way, thinks the whole idea of cosmetic surgery is crazy, and he was supportive even if he didn’t get it. My mom agreed to help me out at home while I recovered too. I met with Dr. Black and we made plans to do a breast lift without implants, a tummy tuck with muscle repair, and Lipo. My surgery was scheduled for 12/3. It’s now 12/6 and I’m 3 days out. I showered for the first time yesterday and as it was the first time I’ve taken off my post surgery bra, I finally saw my boobs. Wow, talk about perky! They are sitting high and are still swollen so I’m sure there will he changes in the coming months. I still have surgical tape on my TT incision and the drain is there as well. From what I can see my tummy is very swollen so I’m not making any judgements yet. Dr. Black has been wonderful. She’s checked in with me even though she’s on a family vacation. Seriously - her office is amazing. Everyone there is kind and compassionate. I do want to mention the pain. There has been absolutely no pain from my breast lift. And I’m serious - NONE. Now my stomach is another animal altogether. Yeow!! The pain is real. Luckily Dr. Black uses exparel to numb you up post surgery. It lasted about 48 hours then the fun began. I feel like a big baby. I knew there would be pain, but had no idea what it would feel like. It’s bad. I stopped taking prescription pain meds after the first day and am taking extra strength Tylenol now. As I still haven’t had a BM I just don’t want to make that any worse. I’ll be taking Miralax this morning and I’ll move around a little more today. I’ll post again soon. Also, thanks to all you wonderful ladies who’ve posted your experiences! Updated on 10 Dec 2019: It’s day 8 and the sun has come out. I am feeling human again and am loving what I see in the mirror. Again, I must say that my boobs are just amazing. They haven’t looked like this since before I became a mom. And I think my tummy is going to be phenomenal. I’my very swollen, but after reading so many of your great reviews, I was prepared for the swelling. Note to all - begin taking a laxative or eat some fiber rich foods as soon after surgery as you can. My fiber of choice - dried figs. I’ve had no trouble in the digestive department. The pain is so much better, and I haven’t even taken Tylenol today. I had my last drain removed yesterday at a follow up with Dr. Black, about whom I can’t say enough good things. She is the real deal. So down to earth and willing to listen and help with any concerns I’ve had. Today I drove carpool, made a quick trip to the grocery and got a simple dinner on the table. I’ll update again soon. Updated on 12 Dec 2019: So today is day 10 and life is good. I ordered a new binder from Amazon and it’s wonderful. I was looking for something softer than the version I came home with, which was scratchy and itchy. The new binder is made of quilted cotton fabric. So much better! I’m getting tired of wearing a surgical bra 24/7, but it’s all for the greater good, so I’ll try not to whine about it. I also found a bra on Amazon that I can tolerate. It’s actually pretty comfy and I’m planning to order another. I am noticing a good amount of swelling beginning midday and continuing through the evening. By morning it is considerably better, but then the cycle begins again. My incisions are healing nicely. Recently I’ve noticed two little sutures poking out, but Dr. Black has already told me to give her a shout and stop by the office if I have any stitches that need to be snipped. If they begin to bother me I’ll give her a call but I think it can wait until my next follow up. She’s instructed me to pat dry after showers and then dab Betadine on my incisions to prevent infection. Also, I’m washing with antibacterial soap. The thought of infection really frightens me so I’ll do my best to follow her instructions to the letter. Well, I’m off to bed - my own bed, thank goodness. I slept in our recliner until last night when I was able to sleep in my bed with pillows under my knees and a big fluffy pillow under my head. I really missed sleeping in bed with my husband! With any luck I’ll be standing fully upright soon. I’m almost there in the mornings, but by nighttime I’m hunched over again. Everything is moving along. Updated on 15 Dec 2019: Here are some before and after photos. I haven’t taken any new ones in a few days, but will soon. I’m just waiting for more of the bruising to subside. Things are looking good and I’m feeling great. Updated on 24 Dec 2019: It’s been 3 weeks since my mommy makeover. The rundown as follows: The positives: I feel pretty good and my energy level is pretty close to what is was pre-surgery. I’m back to work and having no issues - I have a desk job but I’m up and around a lot. I’m super happy with the incisions on my breasts, as they’re healing beautifully. The not terribly bad, but I’m going to get whiney for a minute stuff: I want to soak in a hot bath! I want to sleep without a bra! I hate wearing a binder but I feel much more stability in my core when I wear it. I want to sleep on my side/stomach! I want my scabs to all fall off so I can begin scar therapy! I have about 5 scabs left on my tummy incision and they’re holding on for dear life. So thats it for my rant. Bottom line is that things are going well but I have no patience and this is truly a process. I still feel sore here and there and experience stiffness when I wake in the morning but things improve once I’m up and around. Also the swelling is no joke. By about 1:00 in the afternoon it begins and gets worse throughout the day. I’ll live and I need to get a grip and accept that this is a marathon and not a sprint. Happy healing all you beauties and I’ll update again soon. Updated on 2 Jan 2020: Well, it’s been a whole month already and I can’t believe how fast the time has passed. Things are good. I’ve been back at work for a couple of weeks, am standing straighter every day, have lots more energy, am down from 2 fluffy pillows to 1 sort of flat pillow under my knees when I sleep - it’s more of a mental thing at this point, and I’ve started scar therapy on my tatas. I still have a couple of areas on my tummy incision that I think may be sutures that just haven’t fully dissolved, so no scar therapy for my belly yet. I’m planning to see Dr. Black to have them checked out and removed soon. My incisions all look better, but as I have no patience, I was hoping they’d look even better by now. I’m still experiencing some soreness at all of my incisions but nothing that I even need Tylenol for. I have a pulling sensation in my abdomen sometimes after I’ve overdone it at home. Yesterday after doing some housework I noticed it. These are some photos I took on December 17th and then this morning of the same areas. It’s amazing how the body heals itself. The changes are subtle, but they’re there.
I'll keep you updated as the results appear, but I can at least review the actual process itself. Black's office is great, wait times are low, they're really nice and very real with what to expect. Dr. Black did a consultation and let me know I would be a great candidate for flank Sculpsure. She specified that Sculpsure works best on thinner women with isolated areas of fat. They were great to talk to and I immediately trusted them and was excited to get rolling, which is unusual for me because I've had a lot of unimpressive consults including for Coolsculpting. Less than a week later I had 2 applicators (or 1/2 a full panel) applied on each hip at the same time. They consulted me repeatedly on where exactly I wanted to be reduced on my hips so they could place appropriately. For me, I just wanted the overhanging "bumps" from my love handles to be reduced so that I didn't have double hips when I wear bodycon dresses. The treatment itself was supposed to be somewhat painful, but I took an Advil beforehand and found it uncomfortable but tolerable. Just when I would start to get close to hurting the machine would cycle back to cool. Afterward, they used a massager to dislodge the fat cells, which was uncomfortable but not painful. I was also told I could return to use this massage machine 3 times in the next couple of weeks for max effect. All told I was in and out in less than an hour and back at work feeling zero lingering pain. For aftercare, I will continue to massage the area and drink water. I will update with before/after photos when I have them, but I don't fully expect results until sometime after my second session 12 weeks from now. Nonetheless, the process was nothing to be upset about so if you're worried about pain, take a couple of Advil before like I did! Updated on 24 Oct 2019: I just finished the last post-Sculpsure massage treatment which I'm excited about. The massage treatment is pretty painful but is so fast. I still have no noticeable bruising or soreness unless massaging the area which is great. Updated on 20 Nov 2019: Interestingly today I put on a bodycon dress, the same one that would have bothered me 8 weeks ago, and wasn't really bothered by my love handles. I could still see a slight curve like before, so it's not very visible, but then I noticed my love handles FEEL different to the touch a little bit, and I don't feel them fold over on my waist when I slouch. All small differences, but I also found myself wondering if I wanted/needed a second treatment if my love handles aren't bothering me now. Maybe I'll do my stomach instead? Or maybe both. I will keep updated when I get before/afters and whether I do the second treatment. Updated on 24 Jan 2020: 12 weeks post! I decided to do only one love handle treatment because I didn't want to risk overdoing it or change the shape of my body. I have yet to see the before/afters but I'm fairly sure there's been a slight but appreciated change that only I would notice. So far I would say "worth it" since I didn't need a major change, but I would recommend a different procedure to anyone who would. Since I decided against my second flank treatment, I did a lower abdomen treatment instead. I did right under my belly button, but the person treating me pushed for one right above too, two devices total. The one on the bottom hurt a bit, but the one on my upper abs I felt absolutely nothing at all, making me suspicious that it didn't work properly. I was definitely a bit annoyed that the treatment area was pushed on me and I may not even have had enough fat there to treat or the device wasn't fit properly. I may seek a refund/redo on that area. It's now been a few days and under my belly button is still quite sore when I massage it, there's a little bump which I'm not too worried about. Over my belly button I felt absolutely no tenderness.
Dr. Summer Black is absolutely amazing for several reasons. Dr. Black answered all my questions during my consult and took as much time as I needed. She also encouraged me to call or text her personally with any additional questions I may have thought of after my initial consult. We thoroughly discussed what my “goals” were for having the En Bloc Total Capsulectomy. She was very honest and matter of fact about what I could expect from having this procedure. She did not try to sell me on having additional procedures, even though I specifically asked if I need to have a lift with this procedure (which clearly would have been more expensive), but she told me she did not think I needed it. I have complete confidence in Dr. Summer Black and her father (prior to the surgery and even more so now on the other side). They operate as a team which makes me even more confident knowing there are two wonderful plastic surgeons operating on me. Dr. Black took videos and pictures explaining what she found during the procedure. She called me the evening of my surgery to check on me and we communicated for the next two weeks every day or every other day regarding my recovery. The personal care you receive from Dr. Black Plastic Surgery is excellent (exceeded all my expectations). The results of my surgery are amazing and exactly what we had discussed. If I decide to have additional plastic surgery in the future I would absolutely return to her, hands down. Dr. Black also offers dermal fillers. She injected my lips to increase the fullness. I experienced minimal pain and the attention from the staff was impeccable and I love my lips. Thanks Dr. Black, you gave me a new lease on life!
I researched doctors prior to having my explant as I had been having some issues that were possibly related to my implants not working well with my body and overall health and genetics (namely, several homozygous MTHFR genes). I had a face-to-face consultation with Dr. Black in Metairie on January 7th, and by the end of that meeting I had made up my mind that Dr. Black was the surgeon for me. She's patient, knowledgeable, a terrific communicator, and above all, she didn't invalidate my illness-experience (the things that I had been going through that likely had a component of MY body responding badly to the foreign body that was in it... that is, she didn't rule out that it could be a factor). By the end of the consultation, I knew that she would be the one to do my surgery! I was able to schedule my surgery for two weeks out, and can happily report at this time that I am now breast-implant-free! She did an AMAZING job! My breast already look great, even five days after explant! Additionally, the Crescent City Surgical Center was beyond any expectations I could have had, and the nurses and staff there were EASILY five stars! Due to Dr. Black's "realness", open-mindedness, and willingness to remove my entire implant, and capsule, and to make sure nothing foreign was left in the breast pockets; I will most definitely recommend her to others, and I will definitely make the three hour drive to use her for other aesthetic procedures (skin care, and injectables) in the future. She's absolutely worth it!

patients do occasionally get chronic breast pain with implants. it seems to be more common with implants that are under the muscle. if the implants are the cause of the pain, I would expect that the pain would resolve after the implants are removed. I would recommend finding a board certified plastic surgeon near you and discussing your concerns with them.
I think it would be difficult, if not impossible, to do a true en bloc implant removal under local anesthesia. I recommend finding a board certified plastic surgeon in your area to examine you and discuss your goals. Best of luck.

There is a definite advantage to doing the removal first and the lift after several months. Doing the procedures separately allows the breast tissue time to heal, and retract (lift itself), and re-establish its blood supply that is disrupted during the implant removal and capsulectomy. Without photos or an in person examination, it is difficult to say if you should combine the procedures or do them separately but due to the fact that your implants are so large and your natural breast tissue is so stretched out I would lean toward doing the explant first followed by a mastopexy. Doing the lift at a subsequent procedure 3-6 months later to gives more predictable results and decrease the risk of vascular compromise to the nipple which could result in loss of the nipple. Many plastic surgeons will give a discount on the second procedure even if it is not done at the same time as the first procedure. Hope this helps and good luck.

Thank you for your question. Capsules are the bodies natural response to a foreign object. They are predominately made of your bodies own natural collagen. If you had a thinner capsule, it would likely be resorbed by now since your surgery was 2 years ago. A thicker or calcified capsule has probably not resorbed and can be removed with surgery. Depending on your health problems and what you want, a capsulectomy can be performed when you get these implants removed. Hope that helps and best of luck.
