Hello, I have a breast augmentation coming up in a couple months. The surgeon I chose whom was recommended by a friend does over the muscle breast implants. During my consultation I was told I’ll be getting a lift included. After doing much research I see the majority of patients receive under the muscle. Is there a high risk of complications with over the muscle implants? I’m just concerned on how long they will last and with the cons included, is this a risky procedure? Thanks!
Answer: Lift For my patients who need lift, I place the implants behind the muscle for better long term support and better upper breast fullness. Otherwise, you are asking sagging tissue to support more weight and not sag again, which does not always work.

Answer: Above or below?? Hi Allyssa!Surgeons have their own preferences when it comes to sub-muscular vs. sub-glandular placement of breast implants. Not every patient is a good candidate for sub-glandular placement. The examination will guide the surgeon determine the best approach. If both options are doable, then the surgeon's preference will most likely play a role on the decision making. In my practice, I prefer the sub-muscular approach unless there is a contraindication for this procedure. Some of the reasons I choose sub-muscular placement are: More natural appearance, decreased risk of capsular contracture, decreased risk of rippling and interference with mammograms. We usually go below the muscle in our practice to help distribute the weight of the implant onto the muscle instead of supported by breast tissue alone. In addition, it looks much more natural because the muscle provides covering over the implant so its not as round on the top. I've also noticed the implants drop less over time when they are protected under the muscle.
