We do see some puffiness, which appears relatively mild. In cases like this, a dermal filler can sometimes be used to camouflage the puffiness by blending the surrounding hollow areas. While fillers are temporary, they can give you a good sense of how much improvement is possible without surgery. If the results from fillers are inadequate, then moving forward with lower eyelid surgery may be the next appropriate step.

It is difficult to tell from the photo if you have true fat bulges. If you do then a lower blepharoplasty is a good option. It will reduce the dark and hollow appearance. To make a determination about how to proceed, you're going to need to submit better pictures of your eyelids to make a determination about how best to proceed. If you have herniated fat pads in the lower lids, a trans conjunctival approach, lower eyelid surgery is performed to remove the herniated fat bags creating the puffy look.
