How bad did my breasts bottom out? DO I need to have them fixed? (Photos)

I got a breast augmentation almost 2 years ago. 245cc Natrelle implants. My nipples are very high and I feel like they definitely dropped or “bottomed out”. My surgeon said that I had low skin integrity, but not sure if I have the extra cash to get an internal bra. Does it look like they bottomed out? Does it look like it needs to be fixed or just something I should live with until I need them redone?
