It is very difficult to tell from the one image that you have provided here, but it would appear that in addition to having some outward diversion of your nipples, you also have some asymmetry, or difference in the size and shape of the breasts, and you might even have what we would call "tuberous breast" on the left side. In such cases, we often perform some type of lifting or re-shaping/re-positioning procedure in addition to placing implants as a way to balance symmetry and improve the shape and position of the breasts. I definitely believe that something can be done to help you, although exactly what that would be is best answered with an in-person evaluation by a board certified plastic surgeon who has a lot of experience in all forms of breast surgery. A breast lift procedure (mastopexy) is usually the procedure of choice for nipple repositioning. The tradeoff is visible scarring. In your situation, a lift and small implant (to fill out the upper portion of the breast) could produce a dramatic result. Consult with a board certified plastic surgeon, as definitive recommendations can only be made after a thorough in-person exam and discussion of risks, benefits, and alternatives.
It's pretty difficult to say based on your photo but an in person exam should help and be able to provide a recommendations. It is POSSIBLE to shift the nipples closer to the midline using a breast lift type surgery in some (certainly not all) cases depending on the distance and amount of laxity to your skin and tissue.
