Revision procedures are always more complex than the original surgery, but if you’re struggling to breathe, this may be your best option for relief. While I can’t recommend treatment without a comprehensive in-person consultation, I believe you may need a nasal tip and bridge revision to ensure proper breathing and achieve your desired aesthetic results.
Have you seen your original surgeon to discuss your concerns? This should be the first step when considering a revision rhinoplasty, as your original surgeon possesses the operative report which can sometimes provide information to help understand why your nose is crooked and why you have difficulty breathing. If your original surgeon is unwilling to meet with you (which would be very surprising, as we plastic surgeons should have our doors open to all of our postoperative patients, even from years ago), you can request the operative report, as well as your before and after photos, so that you can consult with another board certified plastic surgeon. It is important to seek surgeons very experienced in revision rhinoplasty. Keep in mind that not all primary rhinoplasty surgeons perform revisions, as the skill set is quite demanding for revisionary surgery. You might require a cartilage graft to help improve your result, but a physical exam would be necessary to make this determination. Revision rhinoplasty is a very technically challenging procedure and should only be performed by the experts, so again, please consult with a surgeon who has decades of experience in revision rhinoplasty.

You need to add back a lot of cartilage: rib is the only source which will give you enough. You should be able to raise your profile back to straight and lengthen the nose. The surgery will be more involved than the primary rhinoplasty but your goals are achievable.
