Dear Megan b,breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. The question of which kind or what size implants to pick is a tricky one that is usually based on multiple factors including the final size you would like to be and your body's measurements. However, gummy bear implants that have a textured surface have been shown to be linked to a low but real risk of a kind of breast-implant associated lymphoma called ALCL. Hi Megan,I have performed many transgender female breast enhancement procedures. Based on my experience, large implants are needed, The original male chest is typically much broader than a female chest and the amount of breast tissue, even with hormone therapy, is usually sparse. So, a different approach is needed.The largest silicone gel (gummy bear) implants available are 800 cc in volume and this is usually around the size we end up choosing. If a larger size is needed, then a saline implant can be used. The saline implants also only go up to 800 cc, but they have been tested and will tolerate up to 3 times that volume. The sterile saline is added to the empty implant shell after it is in position under the breast . And yes, I have several patients with volumes from 1,000 cc all the way up to 2,000 cc. Now, the manufacturer will not warranty an implant that has exceeded its intended capacity, but I also have not had any of these implants rupture or break (so far) in over 30 years of placing these larger implants.I would recommend an in person consultation with a Board Certified Plastic Surgeon who performs these types of procedures frequently. These office visits are very thorough with measurements, photographs, and observing before and after photographs of other patients who are similar in body build and shape to yourself. The procedure is performed as an outpatient and takes approximately one hour, you would need someone to help you in the first 24 hours. No drains are used and the sutures are dissolvable.
