breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Hi and thank you for your question.  The best advice would require an in person consultation and physical exam but it does sound as though you may have developed some scar tissue that is causing you discomfort.  It also seems as though you may require some pocket repair work if your implants are displacing laterally (towards your armpits) depending upon your positioning.  Breast revision surgeries can be some of the more complicated procedures that board certified plastic surgeons perform.  Many surgeons, myself included, perform these revision procedures on patients who need revisions for a variety of reasons: capsular contracture, poor scarring, breast pocket repairs, bottoming out of implants, double bubbles, etc. Considering your surgical history and your desired cosmetic results, an in person consultation and evaluation would be strongly recommended.  After your assessment and discussion of your surgical goals, your surgeon will be best able to accurately advise you.  I hope that this answer has been helpful.  I wish you all the best.
