Hello I am a 5’7 135lb 22yr old female who has undergone a submuscular breast augmentation with mastopexy a year ago. However I think I went too big as my right breast doesn’t have enough tissue to hide signs of the implant. I also feel as if now I appear slightly too “topheavy” as my breasts are more projected than my buttocks. What cost range and recovery could I be seeing if I were to go for fat grafting to my right breast and buttocks area?
Answer: Mini lift I would not recommend fat transfers to your breasts. Your breasts are low on the chest wall and the nipple-areola complex is pointing outward. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. Fat transfers can be performed of the buttocks during a separate procedure. Best Wishes, Gary Horndeski, M.D.

Answer: Cost of surgery Dear Nanna345, the cost of surgery depends on a lot of factors (type of surgery, number of treated areas, anesthesia fee, surgeon, facility fee, city, etc.). It would be the best to visit or call plastic surgery offices and ask for a price. Answer: Breast revision with BBL There may be better options for treating your breast but there’s no reason why you can’t have a BBL. Yes always look for the most experienced talented local board-certified plastic surgeon for each procedure you’re looking for. It made me to be two different procedures to find whoever is best at breast surgery versus fat transfer. For example my practice is devoted exclusively to Liposuction and fat transfer and I do those procedures quite well. I no longer do breast implants because I specialize in only one thing. Most plastic surgeons do general plastic surgery. The key is to get one that has talent and experience and can help you out with what you’re going through. Converting implants to silicon can sometimes improve palpability quite a bit. Sometimes changing the implant contour can help as well. Adding Alloderm as another option. Fat grafting to the breast maybe possible but the amount of fat that survives is directly related to how thick the host tissue is where you need the graft to survive and if you can feel the implant then that tissue was probably very thin. in very thin tissue there is very little host tissue and with very little host tissue very little grafted fat survives. Follow up with the surgeon who did your procedures and if that doesn’t suffice then start scheduling second opinion in person consultations. Delivering consistent quality liposuction and fat transfer results is more difficult than people believe it. The results from these procedures are permanent and more or less irreversible. For that reason I recommend patient schedule multiple in person consultations which time you should ask each provider to show their entire collection of before and after pictures for the procedure you’re interested in. An experienced provider should have no difficulty showing you 50 or more sets of before and after pictures. Seeing a handful of pre-selected example pictures which in fact are probably the best results of that providers career is insufficient to know what average results look like or how many of these procedures the surgeon has performed. The highly experienced surgeon will have hundreds to even 1000s of before and after pictures. This is especially pertinent to Liposuction and fat transfer because the results vary immensely depending on who does the procedure. Good luck, Mats Hagstrom
