I had closed septorhinoplasty 6 weeks ago and very concerned. My columella is very wide and seem to be a cartilage either side protruding and blocking my airways (nose also very narrow). It’s uncomfortable and feels its pushing up and pulling my lip forward/up. My PS says he placed a small cartilage graft so tip doesn’t drop and I need to wait for swelling to go down. Would really appreciate some advice please.

Answer: Columella too wide, nose too narrow and breathing issues Without a full set of pre-op and post-op photos -it's difficult to comment accurately. Based on the limited photos--even though you're early post-op it appears that further aesthetic improvements can be achieved. I concur with your observations, these areas can be specifically improved based on a mutually achievable goals.

Answer: Wide Columella after Surgery Hi there, according to your photos your columella seems still very swollen. Gentle pressure on both sides will help. Any reduction with regards to the width of your columella will also improve your breathing. Ideally you should try to communicate with your surgeon about the progress of your healing nose. Answer: Concern 6 weeks post rhinoplasty Hi and welcome to our forum! The procedure of rhinoplasty involves separation of the nasal bones and cartilages from overlying and underlying tissues, fracturing and reshaping of bone, trimming and reshaping of cartilage. The tissue trauma resulting from this "insult" is significant, resulting in the inflammation of healing, swelling (which may be asymmetric), and firmness which may persist for a year or longer. Swelling internally will temporarily affect the nasal airway. This phenomenon will gradually subside. Some surgeons advocate the use of nasal tapes to control external swelling in the early postoperative period. The nasal tip is notorious for taking the longest time to achieve definition! The photographs you present appear to demonstrate residual swelling which still masks the final result. I recommend continued followup with your surgeon as you are a long way from being healed. Its common to look like this and have some breathing issues after septorhinoplasty.You need to have patience to see results. Columellar struct seems to have been used to erect the tip without dropping, and this needs volume within columella which may look this way at this stage. I normally tell my patients that it will take 18 months to look the way the PS has planned for you. I wish you all the best
