My breasts have always been asymmetrical since puberty but it is much worse after weight loss. I saw a plastic surgeon and he said I have grade 3 ptosis. I read last night about dermal wing mastoplexy - do you think I would be a good candidate for that? My goals are 1: symmetry and normal looking breasts, 2: not saggy breasts, 3: size. My current size is 36dd - one side is popping out of the bra and the other fills cup. I am 51 years old and weigh 156.

Answer: Dermal Wing Mastopexy? Thanks for providing photos! There are a number of options that can help you achieve a very nice result. In my practice, we generally take a two-pronged approach to achieve optimal results:Placement of a silicone gel implant behind the muscle to help provide shape, structure, and upper pole volumeVertical Dermal Mastopexy to help reshape the skin envelope, reduce areolar diameter, and life the nipple/areola to a more optimal positionMy suggestion would be to meet with a Board Certified Plastic Surgeon experienced in breast lift surgery. This is one of the more challenging procedures in Plastic Surgery and so you really need someone who performs a good number of these on a regular basis. I hope that helps and wish you the very best!-Gregory A Buford MD FACSBoard Certified Plastic Surgeon and Author

Answer: Mastopexy candidate Thanks for your inquiry, I think you are an excellent candidate for a mastopexy. Many surgeons including myself will use the "dermal wings" of a mastopexy to preserve volume. Symmetry to me is the most important thing for you, interview a couple of surgeons before proceeding and best of luck.
